Course signup

This is a sign up application form for Complete Vocal Institute.

If you wish to sign up or apply for a CVI activity (an education, a course, open house, or as a test singer) please fill out the form as accurately as possible.

If you work from a Mac then use Mozilla Firefox when filling out the form.

Fill out all mandatory fields marked with a yellow * (fields that are not marked with a yellow * do not necessarily have to be filled out.

Click "Submit" when you have finished filling out the form.

Your sign-up form has been filled out correctly if you receive a message saying "Thank you for your application...".

Are you applying for a 3-year Teacher course?
Then you must answer the questions about your teaching experience indicating the exact hours and years of experience.
Please also indicate clearly if any previous singing or music education you have is at a level of Conservatory, University, Bachelor, or Master degree.
